How do you get started in Eating Disorder Treatment or any Outpatient Program?

Please read instructions below - You must complete ALL forms prior to your first visit:

Once a form is begun, it must be completed in its entirety.  There is not an option to "Save" and return at a later time.


By clicking on the appropriate link (Adult Admission Packet or Child/Adolescent Admission Packet), you will be directed to a secure site.  There are 4 forms in each packet.

When all questions of a form have been completed, scroll to the end of that form and click on "Submit". Clicking "Submit"must be repeated for each of the four forms.

Once we receive your Adult or Child/Adolescent Admission Packet, we will...

    1. Review your health information
    2. Verify your insurance coverage
    3. Schedule an initial assessment with our licensed mental health practitioner and consultation to discuss program placement.   

If you are currently in a inpatient/residential treatment program, above forms will be directed to you-via your treatment team.  A phone appointment will also be required with you and/or family member prior to discharge from your current facility.

 

The following Financial Duty Forms link is to be completed at our office using our lobby computers and e-signature pad. Please arrive at least 15 minutes prior to your first appointment to complete these forms. You will need to bring your driver's license and all insurance cards along with a credit card to keep on file in order for you to keep up with all payments.  This policy is in place so that you can focus on treatment when you come in our clinic, vs having to focus on finances. We accept all major credit cards, cash or cashier's checks as a form of payment--no personal checks, sorry.

Answers to Most Frequently Asked Questions regarding Our Higher Levels of Care/Intensive Treatment Programs

We can provide the care and treatment needed so that you can achieve the successful outcomes that you desire and deserve.  Call us and set up an initial assessment and consultation.  This is required so that our clinicians can determine the most appropriate treatment program and level of care for your condition.  If you are making the decision to obtain intensive treatment, we are here to support you every step of the way.  If you are worried about a loved one, we can help you understand the benefits of our program, but it is essential that your loved one is willing to participate.  We empower our patients to determine the path that best suits them, while healing as individuals.

 

Why should I choose Inner Door Center® over a hospital-based program?

As a free-standing facility, we are able to implement changes and make decisions expediently-- that affect your care-- without the need to go through a corporate bureaucracy. Our space is inviting, warm and intimate which lends itself to healing vs. a cold and sterile hospital atmosphere. Our staff is accessible, dedicated and accomplished. We function as a "team," operating with our hearts and intuitive centers. At Inner Door Center®, we advocate for our patients, integrate a sophisticated and innovative treatment model, while tracking outcomes. Our staff participates in ongoing professional development so we are fresh and current.

 

Insurance is so confusing!  Will my insurance cover higher levels of care/intensive treatment at Inner Door Center®?

>>Answer:

We participate with most all reputable health and managed care plans. We know that insurance can be very confusing and the last thing you want to become overwhelmed with, when receiving treatment is your number one priority. Therefore our staff works with you individually to make sure that you understand your health plan benefits.  As a courtesy to you, the staff at Inner Door Center® completes all verification of eligibility and benefits, pre-authorizations and necessary case reviews. Most of our clients use a combination of insurance and cash for the length of time needed in order to receive the most effective and lasting treatment.

 

Why do I need to obtain medical clearance before entering our higher levels of care along with several other assessments?

>>Answer:

In addition to being accredited by the Joint Commission and adhering to the Michigan Mental Health Code, we follow the American Psychiatric Association Guidelines for Eating Disorders along with the ASAMPPC-2R, ASAM Patient Placement Criteria for the Treatment of Substance Related Disorders - if applicable, which guide our clinical policies.  These policies are designed to keep you safe during our programs and establishes a relationship with our team of professionals should you need further medical, psychiatric or psychological intervention or follow up care.

 

Why must I only work with Inner Door Center®’s treatment professionals while I am in the intensive programs?

>>Answer:

Our intensive programs require coordination of care and can be must successfully accomplished when all practitioners are working together.  We hold regular and frequent case conferences regarding your care, while your insurance company requires continuity of care in order to continue authorization care along with payment for services.  You will be referred back to your originating treatment team following discharge from our programs, and your referring team will be kept in the loop of your care, should you authorize.

 

Why is a deposit required for admission into our higher level of care programs?

>>Answer:

Inner Door Center® could not offer you the higher levels of treatment without securing fees in advance.  Each patient in our intensive programs has a very high staff to patient ratio.  In order to sustain our programs for current as well as future patients, we have found it necessary to require prepayment of all co-pays, deductibles, and non-covered services in advance.  An additional deposit may also be required and other stipulations apply that will be discussed with you prior to rendering services.  This is an investment in your care so that you can focus on treatment.

 

My insurance plan doesn’t cover treatment and/or I have been denied the level of care of treatment that I need. What can I do?

>>Answer:

Following a denial and expedited appeal--and with your authorization--we can assist you by interfacing with your employer’s benefits administrator in the human resources department, along with your company's direct insurance liaison or the Third Party Administrator (TPA). The TPA may be a separate company outside of the insurance company contracted to administer the company healthcare benefit. This interface can facilitate an exception, which would over-ride the insurance denial. This is most successful if your company has over 51 employees or is self-insured (your employer pays the healthcare bill directly vs premiums to the insurance company).  An in-network exception will allow us to bill your insurance company directly for services, while an out of network exception will allow you to submit your Inner Door Center paid receipt to your insurance company for direct reimbursement to you. If a benefit exception is not approved by your employer, and you have exhausted all appeal levels following treatment, we can refer you to specialized attorneys who take cases for individuals who have been denied treatment for their eating disorder. On the other hand, here are reasons why some people choose not to use insurance:
  • More privacy and increased confidentiality
  • You will not have to wait for authorization for coverage.
  • You can extend your treatment as needed, and you do not have to worry about insurance benefits running out before your treatment is complete
  • Medical expenses may be tax deductible
Should you decide to call other treatment facilities that work with insurance, it is in your best interest to ask the following:
  • What type of deposit is required in order to enter treatment?
  • What would my financial responsibility be in the event insurance provider denies coverage before I complete treatment?
  • What happens if I do not have the funds to pay the remaining costs? Will I be discharged from treatment?
  • What is the full cost of treatment and are there any potential additional costs (Detox fees, physician fees, psychologist/psychiatrist fees, medications, lab work and specialty services/testing)?
